1. Brown Turkey and the sausage meat separately. Mix the Turkey and sausage meat. Season with salt and pepper. Heat oil in a sauté pan and cook the Turkey mixture until cooked thoroughly. Reserve.
2. Add garlic and onions to the pan and sauté for 1 minute until caramelized. Add the zucchini and tomatoes. Sauté another minute and add the Turkey mixture. Simmer for 2 minutes over medium heat.
3. Brush the polenta discs with olive oil and sprinkle with Parmesan cheese. Bake at 350° F until golden brown. Remove and reserve.
4. To serve, spoon the Turkey mixture over the polenta discs and sprinkle with Fontina cheese. Bake until melted. Garnish with cracked black pepper.
